Community Health Network of Washington (CHNW) offers a variety of health services to address your patients’ whole health. We have free programs available to members with complex health conditions, and connect members with community resources to address everyday factors that affect their health.
Our Health Services team consists of both clinical and nonclinical staff. We will coordinate with you and your patients to create a good care plan.

Good health starts with a good plan of care. CHNW’s care coordination team includes care managers who will help members develop a health management plan with their specific health needs and goals in mind. These programs support the relationship between you and your patients and track how well the care plan is working by using clinical and evidence-based guidelines.
Case management is available to all CHNW members. It can be very helpful for members with complex or chronic medical conditions, often those who are receiving care for more than one medical condition. A case manager will be assigned to help the member maintain and improve their health. You can refer patients as a clinic or provider, or they may refer themselves.
Our Health Coaching program teaches members how to manage their chronic conditions. The program looks for patients with a diagnosis of diabetes, congestive heart failure, asthma, and/or chronic obstructive pulmonary disease (COPD) and engages them to see if they need extra help.
Health is more than just being able to see a doctor. It’s about having access to anything you need to live your everyday life. Our goal is to make sure our members’ basic needs are met. We support them by finding services and other programs in their communities. If your patients need help looking for employment, transportation, food, or long-term housing, our team can support you.
A Health Assessment is a brief survey. It looks at members’ current health and lifestyle and identifies specific health needs. Our Health Management staff uses the results to create care plans and match members with health and wellness services.
